What documents should I submit with my passport application to get a passport in my new name?

You will need to apply for a new passport. If you are:

Renewing your passport

You need to submit:

  • Your previous passport
  • Two documents that show you are using your new name, such as your driver's licence, college identification card, school report, social welfare receipts, pay slip, utility bills, bank statements or official correspondence from a public or private sector organisation.

These examples must date back at least two years from the date you submit your application form.

For name changes concerning children under 18 years, their long form birth certificate must also be submitted.

Applying for the first time

You need to submit:

  • Your long-form birth certificate
  • Your civil marriage certificate or civil partnership certificate (if applicable)
  • The long-form birth certificate and civil marriage certificate of your Irish-born parent (if applicable)
  • Copy of your photo ID
  • Proof of name
  • Proof of address
  • Two documents that show you are using your new name, such as your driver's licence, college identification card, school report, social welfare receipts, pay slip, utility bills, bank statements or official correspondence from a public or private sector organisation.

These examples must date back at least two years from the date you submit your application form.

If you want to change your name following a marriage/civil partnership or if you want to revert to your maiden name other documents are accepted.
