Paper Passport Application Forms

Apply Online

This is the fastest and cheapest way to get your first Irish passport or renew your passport. Once you have your photo, it only takes about ten minutes!

You cannot apply online if you have already submitted a paper application.

  • Application forms cannot be downloaded.
  • Forms are available in Irish and English.
  • If you cannot get a form or need a large number of forms, contact us and we will post them to you.

How to get a paper application form

You need to get an APS 1 Form from one of the following:

  • A Garda Station
  • A Post Office
  • Passport Office in Dublin or Cork

You must submit your postal application in the country you live in. For example, if you live in the USA and are on holidays in Ireland, it is not possible to apply by paper application form from Ireland.

Sole Guardian Affidavit (AFF1) Form

Passport applications for children under the age of 18 need the consent of all guardians. In certain circumstances, a passport application for a child may need to be accompanied by a Sole Guardian Affidavit Form.

Declaration Form A

What is it

A sworn statement by the parent, giving details of their period of residency in Ireland.

When do you need it

  • The child was born after January 1st 2005 and
  • You want to show the citizenship of the child through habitual residency in Ireland and
  • The parents were born within the European Union (EU), European Economic Area (EEA) or Switzerland (Excluding Northern Ireland and Great Britain).
You must get this form witnessed.
